You need to have valid, current photo identification when you are tattooed or pierced. The reason is this. You will be asked to read, understand, and sign a release form prior to the service being rendered. This is a legal contract. We have to show due diligence, to prove you are who you say you are, when obtaining your signature.
We MUST make due effort to verify you are who say you are for the contract to be binding.
If you are using a knick name or alias, or you are a minor, the contract may not be enforceable. We need to show effort to verify this isn't happening. (I hate all this legal stuff but it is required).
All ID must be current, have a photo of you, and your date of birth. If you're a minor, you gotta have PHOTO ID and so does your Legal Parent or Guardian. If you don't have the same last name (like your Mom got remarried) be prepared to prove they are your legal guardian. If you can't, no tattoo or piercing. Also, keeping pace with a new state law, your parent or guardian must remain with you during the procedure. No more "sign and leave" for the parents. If for any reason, we feel that your ID does not meet our requirements, you're outta luck, no procedure will be performed.
No, your big brother, sister, uncle, grandmother, pet hamster can not sign for you. If you're under 15 don't even call, you'll just be wasting your time...sorry but that's just the way it is......

No matter what ID you have...it MUST HAVE a photo of you, and your birth date and BE CURRENT.
